Voter Registration Form

The Florida Voter Registration Application Form is used to register to vote in the State of Florida. The application form cannot be copied, faxed, or downloaded because the law requires the registration information to be entered on an original voter registration application form. You can complete a request to have an original Florida Voter Registration Application Form mailed to you by the Supervisor of Elections Office or you may obtain one from the locations listed in How to Register Section.


Keeping Your Voter Record Current
After you are registered to vote, it is important to keep your voter registration record up to date with your current address, name, and desired party affiliation. You can update your voter registration records or replace your defaced, lost or stolen voter identification card by sending a written, signed request to the Supervisor of Elections office at the address shown at the bottom of this page. A new voter identification card will be mailed to you approximately one to two weeks after your request is processed.
